The DM decides weather a character is in a good position to notice something. If you must be within 5 feet of a secret door to have a chance, and you never go near it, the passive perception does not matter.

Some DMs treat passive perception as a minimum value. A character that is not unconscious or petrified cannot reach a lower result even with an active roll. If a character has really high perception, you commonly do not need to ask for a roll for clear reasons.
